Mental Illness in Speculative Fiction

Mental illness was one of the hallmarks of 19th century Gothics, from mad wives chained in attics to R. M. Renfield eating flies in the asylum. More recently, there’s been a preoccupation with using vampirism as a method of discussing addiction, or using addiction as a model to describe vampirism. Has our modern understanding of mental illness changed how we depict it in speculative fiction – and should it?
